Each lost extremities after being captured by their enemies, save Morgoth:

Huor - head hewn at the close of the Nirnaeth
Maedhros - bye bye, hand
Azog - beheaded by vengeful dwarves
Gelmir - dismembered in front of his brethren by Morgoth's minions
