I think it likely Sauron sensed Gandalf was more than a wandering old man, being a kindred spirit himself. At that point in the rebuilding of his power, he would rightly have been wary of his true nature being learned by the Wise and a 'strategic retreat' was a prudent choice. That would explain as well why Gandalf thought to disguise himself on his return to Dol Guldur.

He may have determined that at least the interloper 'felt' like the one who had intruded before, and probably assumed he was a spy of the Wise. The words of the Mouth of Sauron to Gandalf show that Sauron was well aware of Gandalf, and probably had him pegged as an emissary from the West, since he plainly was not mortal, and did not appear to be an Elf. Exactly when he decided that though, is naturally conjecture.

I think Sauron's hold on the Men of the East was pretty secure in the Third Age. There was ample record of their attacking Gondor off and on, from the early years of it. The Nazgûl were said to have fled east after Sauron's defeat at the hands of Elendil and Gil-galad, and they would have fomented the fear and devotion of him there until he was sufficiently able to resume domination on his own.
