Angmar isn't the name of the Witch-king. It is just the name of his realm. He is always referred to as the "Lord of the Nazgûl", or the "Witch-king of Angmar". No one knows any of the Nazgûl's names (save Khamûl and *possibly* Gothmog [see other threads for that]).

But the rest is correct.
