From the Complete Guide to Middle Earth:
Elessar : (Q.: 'elf-stone') The name under which Aragorn (q.v.) took the throne of the Reunited Kingdom. It was foretold for him by Galadriel, and was given him by the people of Gondor during the WR because of the great emerald brooch, a gift from Arwen, which he wore.

I dont know if this is truly correct, but its what i found. If im wrong, I apologise.
