I am willing to go with the Atlas of Middle Earth on this. Henneth Annun was gouged out by the stream that was flowing over the falls. The men of Gondor worked the stream to flow over its old course over the falls and sealed off the back, creating the ultimate hideout.
This was always one of my favorite places when reading the book. The description of the window to the West and the sun really filled my mind with a beauty unmatched.

When Elendil fell, Isildur departed to take up the high kingship of his father, and committed the rule of the south in like manner to the son of his brother. He did not relinquish his royalty in Gondor, nor intend that the realm of Elendil should be divided forever.
